Apply for Youth Programs at LightHouse SF

All students that wish to participate in LightHouse Youth Program must complete an intake process. To start the intake and applications process please apply for Youth Programs by completing the below Student Intake form.

General Student Intake Form can be found here.

Once intake form is complete a member of the youth services team will reach out to complete the intake process which may include the following forms: Waiver and Liability Release Form, Student Rights and Responsibilities, Self-Disclosure and Medical Insurance Form and Student Behavior Contract.

If you have already completed these forms in a previous year and would like to register for new programs, contact youth@old.lighthouse-sf.org to update your information and program preferences.
